For new patients, please complete this, print it out, sign it, and bring it with you to the clinic. If you don't have quick access to a printer, we'll provide you with hard copies at the front desk.
If you're coming in for a follow up visit and your previous visit was in the last few days, there's no need to re-do paperwork. But if you've been to our clinic before (and it's been a while) and you are returning, welcome back! All you need to sign is our financial policy. The front desk will have a verification of address for you.
